I think I have to ask you to provide us with the HBA logs too to know for 100% what is going wrong there. Is it possible?
If this is not possible for some reason then I will ask you get a dump with Wireshark on port 3260.
Will it be OK for you?

I'm pretty much sure there are no logs with HBA. Use Wireshark to trace iSCSI handshake.

I can not seem to figure out HOW to capture the iSCSI traffic with Broadcom BCM509C dual port controller with iSCSI offload
The card is seen as dual NIC (NDIS driver & IP), but for iSCSI it uses completely different IP (assigned in BACS)
The iSCSI part is not visible in Wiresghark

As I said, in this screen one can ONLY see NDIS adapters
iSCSI HBS does NOT use NDIS driver, hence it is NOT visible to select

You need to try collecting WireShark logs @ server side in such a case.
