Licensing for 3 HA hosts and replicas?

I'm licensed for HA with 2 hosts. How does licensing now work with the 3 host capability with V6? Do you sell separate two and three host licenses, or does my license "upgrade" to be able to use 3 hosts?
Also, is replica part of the base licensing, or is that separate?
