I have a newly built starwind server put into production last month December. Twice now out of the blue the Starwind Service would stop rendering all luns unavailable. A simple restart service brings all functionality back, In the logs I see a lot of entries saying "PDU is dropped: CmdSN 0 is out of range (ITT 0x0)
What could be the cause of this.

StarWind version? What do you use to connect to target? What configuration do you have? HA non-HA? Do you happen to have a complete log?
iSCSI sends commands numbered (incremented) so there's no way initiator will send 0 cmd again. Something went wrong (session disconnected by initiator but not physically dropped by TCP stack for example). Very strange. Need log to figure out what's going on.

Starwind is Version 6 running in Non HA mode on Windows 2008R2 connecting to ESX 4.1. I have emailed in the logs.

Thank you. I will download it and install.
In the meantime can you tell me what you saw that was wrong with my setup. Another system built a day before or a day after this system is working fine with the same specs and same setup.

There was some issues with iSCSI commands processing in some cases, and you faced on of such unfortunately.
